Appointment Cancellation/No Show Policy

Thank you for choosing Suburban Physical Therapy & Sports Medicine Center, LLC. When you schedule an appointment with our facility, we set aside ample time to provide you with the highest quality of care. Should you need to cancel or reschedule an appointment, please contact our office as soon as possible, and no later than 24 hours prior to your scheduled appointment. This gives us enough time to schedule other patients who may be waiting for an appointment. Please read our policy below:

  • Effective November 1, 2016 our no show fee is $45.00. This fee is your responsibility and your insurance will not be billed.
  • If you fail to show for your Initial Visit and you want to reschedule, it will be scheduled the following week when time allows.

We understand there may be time when an unforeseen emergency occurs and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances, please contact our office as soon as you can. You can contact our office 24 hours a day, 7 days a week at 860-721-9801. Should it be after regular business hours Monday through Friday, you may leave a message on our answering machine.
